How to Review a Medical Referral for Missing Information

Use the receiving service's checklist

Requirements vary by specialty, organisation and referral pathway. Common administrative items can include patient identifiers, contact details, referring provider, referral reason and expected supporting documents. The receiving service should define the authoritative checklist. Avoid using a generic online list to reject or delay care.

Make the document readable

Digital PDFs may contain searchable text, while faxed and scanned referrals often require OCR. PDF Guard can read both locally in the browser. Check names, dates of birth, record numbers and contact details against the page because OCR can substitute similar characters. A low-quality scan should be clarified through the approved process.

Treat missing as a question

A not-detected result can mean the field is absent, written under another label, handwritten, contained in a separate attachment or misread by OCR. Open the original, inspect the relevant area and follow the receiving organisation's procedure. Do not interpret the automated label as a clinical judgement or a reason to ignore urgent information visible elsewhere.

Keep clinical decisions outside the tool

PDF Guard does not diagnose, assess urgency, interpret test results, recommend treatment or decide whether a referral is appropriate. It cannot validate patient identity or clinician credentials. Authorised professionals must make all clinical and access decisions using approved systems, complete records and their judgement.

Handle the report carefully

The current checker keeps the referral and extracted text in browser memory and does not send them to an external AI model. A downloaded report may contain administrative findings even though it omits the full text, so store it only where authorised. Closing the tab clears the working session; it does not delete files you chose to download.
